The name you wish to have on the certificate, i.e., John Doe or The Doe Family.
Date of the Launch you witnessed.
Your Viewing Location (in general terms), i.e., Highway 528 near Port Canaveral, Jetty Park, Cocoa Beach, Titusville Space View Park, et.
Your Email Address.
